P.S. 211 Registration for the Upcoming School Year

 

Please be advised that P.S. 211 will be open for new student registration during the last week of August. We invite parents to view the information on this page and come visit us to register your child at our school (more information can be found on the flyer below)!

 

We are excited to announce that we are going to host a P.S. 211 Open Enrollment Event for any parents that would like to register their child to attend our school in any of the grades that we support (Kindergarten all the way through 5th Grade) for the 2024-2025 school year! Registration Information & Process

 

Please be sure to look through and review all of the information on this page before physically visiting our school so that we can get your child registered in an efficient and timely manner.

 

Process Overview

 

Parents and guardians looking to register their child in an NYC school will need to first gather essential documentation/information to bring to their school of choice and fill out additional forms that the school requires to complete the registration process.

Personal Documents
 
The first step in registering your child at a new school is to gather all of the required documentation that you will physically need to bring to our school when you visit us on the designated “New Student Registration” days (please view our latest “New Student Registration” Flyer towards the top of this page for more information).

The documents that you will need to bring with you when you visit the school are as follows:
 
  • Your Child’s Birth Certificate
  • A Proof of Address
  • Immunization Records
  • Parent/Guardian Identification Card (I.D.)

Parents and guardians will also need to fill out various forms when registering their child to a new NYC school.


Some of these forms can be filled out digitally using the various links below each section (please do your best to fill out as many of these forms prior to your visit) while others will need to be filled out during your visit to the school.

  3. Student Allergy Information FormThis is available as a Google Form in the preceding link or the links section below
  4. Federal Parent/Guardian Student Ethnic & Race Identification FormThis is available as a Google Form in the preceding link or the links section below
 

 

Forms That Have to Be Filled Out During Your Visit to the School:

  1. Provisional Admission FormThis form will need to be filled out and signed during your visit to the school.
  2. Consent to Photograph, Film or Videotape a Student for Non-Profit UseThis form will need to be filled out and signed during your visit to the school.
  3. Walking Field Trip Permission SlipThis form will need to be filled out and signed during your visit to the school.
  4. Housing QuestionnaireThis form will need to be filled out and signed during your visit to the school.
  5. Student Health Examination FormThis form will need to be filled out by the parent and their child’s primary doctor. Please either download/print the form or visit us to receive a physical copy and take it with you to your child’s doctor. Once filled out please return a physical copy to us.
