P.S. 211 Registration for the 2022-2023 School Year
Please be advised that P.S. 211 will be open for new student registration during the last week of August. We invite parents to view the information on this page and come visit us (on the days listed below on the flyer) to register your child at our school!

Process Overview
Parents and guardians looking to register their child in an NYC school will need to first gather essential documentation/information to bring to their school of choice and fill out additional forms that the school requires to complete the registration process.

The documents that you will need to bring with you when you visit the school are as follows:
- Your Child’s Birth Certificate
- A Proof of Address
- Immunization Records
- Parent/Guardian Identification Card (I.D.)

School Forms
Parents and guardians will also need to fill out various forms when registering their child to a new NYC school.
Some of these forms can be filled out digitally using the various links below each section (please do your best to fill out as many of these forms prior to your visit) while others will need to be filled out during your visit to the school.
The forms that parents and guardians will need to fill out to fully complete your child’s registration are as follows:
Forms That Can Be Filled Out Digitally:
- Student Information Card (Blue Card) – This is available as a Google Form in the preceding link or the links section below
- New Student Registration Form – This is available as a Google Form in the preceding link or the links section below
- Student Allergy Information Form – This is available as a Google Form in the preceding link or the links section below
- Federal Parent/Guardian Student Ethnic & Race Identification Form – This is available as a Google Form in the preceding link or the links section below
Forms That Have to Be Filled Out During Your Visit to the School:
- Provisional Admission Form – This form will need to be filled out and signed during your visit to the school.
- Consent to Photograph, Film or Videotape a Student for Non-Profit Use – This form will need to be filled out and signed during your visit to the school.
- Walking Field Trip Permission Slip – This form will need to be filled out and signed during your visit to the school.
- Housing Questionnaire – This form will need to be filled out and signed during your visit to the school.
- Student Health Examination Form – This form will need to be filled out by the parent and their child’s primary doctor. Please either download/print the form or visit us to receive a physical copy and take it with you to your child’s doctor. Once filled out please return a physical copy to us.
