Student & Parent Contact Information Update (Blue Card)

 

Contact Information Update


Parents wishing to update their child’s information or their own contact information can find all of the necessary information below. Currently, there are two places where parents have to update their child’s information along with their own contact information:

 

  1. Filling out our digital “Student Blue Card” Google Form
  2. Updating any/all contact information within their child’s NYC Schools Account

The Importance of Having Up-to-Date Contact Information


Ensuring that you and your child’s information is up-to-date is one of our top priorities and really important for your child to be safe and healthy while attending our school. 


Help us by keeping your information as updated as possible by filling out both the digital “Student Blue Card” Google Form and updating your information on your child’s NYC Schools Account. This will allow us to successfully contact you in the event of an emergency or if we need to provide you with any important updates.
 
 
 
 

Updating Your Contact Information Has Never Been Easier!


We have made it very easy for our parents to update their contact information no matter where they are! As mentioned above, parents have to update their child’s information along with their own contact information in two places: 

  1. The digital “Student Blue Card” Google Form  
  2. Their child’s NYC Schools Account
 

Updating our school’s “Student Blue Card” Google Form allows us to successfully contact parents in case of an emergency while also allowing us to communicate out any important information/updates.

 

Keeping your child’s NYC Schools Account updated allows the NYC Department of Education to successfully keep other systems which require parent’s contact information (such as bussing and routing information which falls under the Office of Pupil Transportation) while also allowing the NYC Department of Education to communicate out any important information/updates.

 

 

Student Blue Card Google Form Contact Information Update


Most parents may already be used to filling out “Blue Cards” for their child as it is a requirement in every NYC school. We have made it easier than ever for our parents to fill out a “Blue Card” for their child digitally using an easy-to-use Google Form.


Our digital “Student Blue Card” Google Form can be accessed and filled out using any device and has been designed with efficiency in mind. The Google Form is divided into various sections (which can be skipped if they don’t apply to you) and includes all of the same questions/information that you would find on a physical school “Blue Card”. 


Please answer all of the questions to the best of your abilities and if any changes have to be made you can fill out the Google Form again or contact our school’s Pupil Accounting Secretary.

NYC Schools Account Contact Information Update


Parents also have to update their contact information on their child’s NYC Schools Account. Parents can create or log into their child’s NYC Schools Account using the link provided and access a variety of different information while also being allowed to update their “Emergency Contact Information”.


Please be advised that the information stored on this account is separate from the “Blue Cards”, but it is imperative that our parents update both systems whenever necessary.

Other Digital & Physical Forms Are Available:


Parents can also access these forms (along with other important forms) online in their PDF versions which allows parents to either fill them out digitally or print a copy of them to fill them out physically. 



These forms (which are normally required during New Student Registration) allow parents to update information such as allergy information, housing information and their child’s health information.


These forms (along a few others) can also be accessed as PDF files and printed as necessary (via the Google Drive Folder Link or through the NYC Department of Education Official Enrollment Forms Website - both linked here and below).
