Dear families and staff of P.S. 211 Elm Tree Elementary School,

We would like to take this opportunity to inform you that the Office of Food and Nutrition Services has created a survey for students/parents regarding our school’s food menu and they would like YOUR feedback!

The Office of Food and Nutrition Services wants to provide you with the best food service program possible. This is why they have set up a survey using SurveyMonkey to collect feedback regarding our school’s food menu.

The survey can be accessed using the link below or scanning the QR code on the flyer at the top of this page:

Parents and students are urged to answer the several questions on the survey in order to provide their feedback. The survey can be easily translated using the dropdown menu at the top right corner of the survey itself.
